This Publication Scheme provides a guide to the services and practitioners who practice together within the Nork Clinic General Practice as required by the Freedom of Information Act 2000. It is based on the model publication scheme for general practices produced by the NHS Freedom of Information (FOI) project board and the British Medical Association. PART ONE: Introduction The publication scheme is a complete guide to the information routinely made available to the public by Dr. Masood Ahmad, Dr. (Mrs) Nuzhat Ahmad and Dr.Kantilal G. Patel. It is a description of the information about the Nork Clinic Practice and the General Practitioners that work there that is available to the public. It will be reviewed at regular intervals and we will monitor its effectiveness. Your rights to information _ The Freedom of Information Act 2000 recognises that members of the public have the right to know how public services are organized and run, how much they cost and how the decisions are made. _ From January 1st 2005 the Freedom of Information Act will oblige the General Practice to respond to requests about information that it holds, and is recorded in any format and it will create a right of access to that information. PART TWO: CLASSES OF INFORMATION All information at the Nork Clinic General Practice is held, retained and destroyed in accordance with NHS guidelines. This practice aims to follow national and local best practice guidelines, including the NICE best practice guidelines and the National Service Frameworks. (Please see attached document) The practice is open at the following hours; Weekdays 8.30 a.m. - 6.30 p.m. Saturdays closed all day Sundays closed all day Out of hours cover is provided by the Thamesdoc GP OOH Service between the hours of 6.30pm-8.30am on weekdays, all day on Saturday and all day on Sundays and Bank Holidays. The telephone number for Thamesdoc GP OOH Service is 0208 390 9991. Members of staff conduct consultations in English. Drs. Ahmad and Patel also speak fluent Urdu. If translation services are required please contact us in advance. An interpreter for people with hearing difficulties can also be provided through the PCT. Dr. M. Ahmad has special interests in child health, surgical aspects, gastro-cardiology Intestinal problems, kidney, bladder, diabetes and asthma disease. Minor Surgery is carried out at the practice as well as a regular verruca clinic. Dr Ahmad also undertakes some minor surgery for other local practices, including vasectomy. Dr N Ahmad has special interests in womanÕs health, providing a full range of maternity, well woman and family planning services, Dr K Patel has a special interest in the management of asthma, anxiety and depression. Dr V Moorthy has a particular interest in dermatology. Dr. Sally Hudson has a particular interest in diabetes and the menopause.
