Quality and Outcomes Framework
This is a quality framework through which GP surgeries across the country are assessed across a wide range of clinical and organisational indicators.
We were very pleased to have achieved the joint top score in Surrey PCT for 08/09 with a maximum of 1000 points.
Patient Confidentiality and Access to Medical Records
We respect your right to privacy and keep all your health information confidential and secure in accordance with the Data Protection Act 1998.
You have a right to access your medical records in accordance with the Access to Records Act 1990. Any request for a report or copies of notes requires a signed patient consent form and will incur a charge.

If you have any suggestions or issues that you wish to raise please ask at reception for more information.
Complaints Procedures
We offer a practice complaints procedure to deal with comments, suggestions and complaints about the service we provide. Our practice manager will give you further information. Our aim is to give you the
highest possible standard of service and we try to deal swiftly with any problems that may occur.
Violence/Abuse
The practice has a Zero Tolerance approach to any forms of abuse whether verbal or physical. We reserve the right to remove patients from our practice list should there be an irrevocable breakdown in our
relationship with them.

Education
Our practice is committed to developing the skills of medical students. We frequently have them attached to our surgery. You will be informed if a medical student will be sitting in with the doctor or seeing
patients themselves. Please feel free to decline should you wish not to be seen by a medical student.
Change of Personal Details
If you change your name, address or telephone number please give full details at reception, including your postcode. If you move outside the practice area, you will be asked to find a
doctor in your new area. Primary Care Support Services can be contacted on telephone number 020 8335 1400 and they will help you if you experience any difficulty.
Sickness Certificates
You do not require a doctor's sickness certificate for any illness lasting seven days or less. Your employer may ask you to complete a self-certification form (SC2), which is available from your
employer. For any illness lasting longer than seven days, you will need to see the doctor for a sickness certificate (MED3).
Home Visits
Please come to the surgery whenever possible, as facilities are better for examination and treatment. If you think you need a home visit, please telephone the surgery before 10.30am and give the receptionist
as much detail as possible so that the doctors can plan their rounds. It may not always be possible for your own doctor to visit.
