New PatientsHow To Register With The SurgeyOur practice list is open and we are registering new patients every day. The registration process is quick and simple to access. Patients wishing to register with this practice will need to complete a registration form which can be downloaded below or picked up from reception. You will also need to provide identification documents (ideally passport) and a recent (three months) utility bill; unfortunately we cannot accept mobile telephone bills. You will also be asked to complete a new patient questionnaire (this too can be downloaded below) and will be invited to attend for a short health check. This will help us identify any current problems and gives new patients an opportunity to ask relevant questions. Please bring a urine sample with you. You will be officially registered with the practice and may therefore see any doctor in your allocated surgery. We currently accept patients from Banstead, Burgh Heath, Kingswood, Langley Vale, Nork, Tadworth, Walton-on-the-Hill and Woodmansterne.
